Yes. A person can have diabetic peripheral neuropathy even when their blood sugar currently appears to be well controlled.
That can be confusing.
Patients sometimes tell me:
“Doctor, my A1C is good now. Why are my feet still numb?”
The answer is that nerve health reflects more than a single blood test.
Peripheral nerves may have been exposed to abnormal glucose levels for years before symptoms developed.
And once nerve damage has occurred, improving blood sugar does not necessarily cause symptoms to disappear immediately—or completely.
But this is extremely important:
Good blood-sugar management still matters.
Improving glucose control may help reduce the risk of additional nerve damage and other diabetes-related complications.
The fact that neuropathy hasn’t disappeared does not mean your efforts are wasted.
How Does Diabetes Damage Nerves?
Diabetic neuropathy is complicated.
It is not simply a matter of “too much sugar touching the nerves.”
Long-term metabolic changes associated with diabetes can affect nerve cells and the small blood vessels that help supply those nerves.
Over time, this may interfere with:
- Normal nerve metabolism
- Blood supply to nerves

- Electrical signaling
- Nerve structure and function

What Does Diabetic Neuropathy Feel Like?
There is no single diabetic neuropathy symptom.
Patients may experience:

- Tingling

- Stinging
- Electric sensations
- Sharp or shooting pain
- Increased sensitivity
- Reduced temperature sensation
- A feeling of walking on padding or rocks
- A sensation of wearing socks when barefoot
- Balance problems
Some people have significant nerve damage with surprisingly little pain.
That is one reason absence of pain does not necessarily mean absence of risk.

This is one of the most common points of confusion.
A1C is an extremely useful test.
It provides an estimate of average blood glucose over roughly the previous few months.
But your peripheral nerves have a much longer memory than that.
Imagine someone who had elevated glucose for many years and then dramatically improved their diabetes management.
Their current A1C may look excellent.
That is wonderful.
But today’s A1C cannot erase years of previous metabolic exposure.
The current number tells us something important about where you are now.
It doesn’t tell the entire story of where you’ve been.
A1C Is Important—but It Isn’t the Entire Picture
Two patients can have the same A1C and still have different glucose patterns.
For example, one person may have relatively stable glucose throughout the day.
Another may experience larger highs and lows that average out to a similar A1C.
That doesn’t make A1C unimportant.
It means it should be interpreted as part of the larger clinical picture.
Depending on the patient, healthcare professionals may also consider:
- Fasting glucose
- Home glucose readings
- Continuous glucose monitor data
- Duration of diabetes
- Medications
- Kidney function
- Blood pressure
- Cholesterol and triglycerides
- Weight and metabolic health
- Smoking history
- Physical activity
Diabetes is a whole-body metabolic condition.
Neuropathy should be viewed in that context.
How Long Have You Had Diabetes?
Duration matters.
In general, the longer someone has diabetes, the greater the opportunity for complications to develop.
But another important question is:
How long was abnormal glucose present before diabetes was diagnosed?
Type 2 diabetes often develops gradually.
A person may experience years of insulin resistance and worsening glucose regulation before receiving an official diagnosis.
That means the metabolic process may have been affecting the body long before someone first heard:
“You have diabetes.”
Can Prediabetes Be Associated With Neuropathy?
Some people with impaired glucose regulation or metabolic dysfunction can experience neuropathic symptoms even before they meet the diagnostic criteria for diabetes.
This is one reason I don’t like to think only in terms of:
“Diabetes: yes or no?”
Sometimes the better question is:
“How healthy is this person’s glucose metabolism?”
However, burning or numb feet should not automatically be blamed on prediabetes.
Other causes still need to be considered.
If I Have Diabetes, Does That Mean Diabetes Caused My Neuropathy?
Not necessarily.
This is another important point.
Diabetes is a common cause of peripheral neuropathy.
But a patient with diabetes can still have:
- Vitamin B12 deficiency
- Nerve compression
- Lumbar spinal problems
- Thyroid disease
- Kidney disease
- Medication-related neuropathy
- Chemotherapy-related neuropathy
- Alcohol-related nerve damage
- Other neurological disorders
Sometimes more than one factor is present.
So even when a patient has diabetes, I still believe it’s reasonable to ask:
“Is diabetes the entire explanation?”
What About Metformin and Vitamin B12?
Metformin is widely used to treat type 2 diabetes and can be very beneficial.
However, long-term metformin use can be associated with reduced vitamin B12 levels in some patients.
Vitamin B12 deficiency can itself contribute to neurological symptoms and neuropathy.
That does not mean patients should stop taking metformin.
It means B12 status may be worth evaluating in appropriate patients—especially when neuropathy or anemia is present.
Medication decisions should be made with the healthcare professional managing your diabetes.
Can Better Blood-Sugar Control Reverse Neuropathy?
Sometimes symptoms may improve, particularly when contributing factors are identified early and addressed.
But complete reversal cannot be guaranteed.
Once significant nerve damage has occurred, recovery may be:
- Slow
- Partial
- Variable
- Incomplete
This can frustrate patients.
They may work hard, improve their diet, take their medication, lower their A1C—and still have numb feet.
Please don’t interpret that as failure.
Improving diabetes management has benefits far beyond whether your toes immediately regain normal sensation.
It may help reduce the risk of further complications involving the:
- Nerves
- Eyes
- Kidneys
- Heart
- Blood vessels
Good metabolic management remains worthwhile.
Why Foot Protection Is So Important
One of the greatest dangers of diabetic neuropathy is not necessarily pain.
It is loss of protective sensation.
If you can’t adequately feel your feet, you may not notice:
- Blisters
- Cuts
- Burns
- Pressure areas
- Objects inside a shoe
- Skin breakdown
A small injury can become a much larger problem if it goes unnoticed.
Make Foot Inspection a Habit
For patients with significant sensory loss, regular foot inspection is important.
Look for:
- Redness
- Blisters
- Cuts
- Cracks
- Swelling
- Drainage
- Pressure areas
- Skin color changes
If you cannot easily see the bottoms of your feet, a mirror or help from a family member may be useful.
Report concerning wounds promptly.
Shoes Matter
Reduced sensation means the foot may not warn you that a shoe is creating excessive pressure.
Choose footwear that:
- Fits properly
- Has adequate room
- Does not create rubbing or pressure points
- Protects the feet
Check inside shoes before putting them on if sensation is significantly reduced.
And be cautious about walking barefoot.
What About Circulation?
Diabetes can affect both nerves and blood vessels.
A patient may have:
- Neuropathy
- Peripheral artery disease
- Or both
Poor circulation can interfere with healing and increase the seriousness of foot injuries.
Symptoms such as wounds that heal poorly, significant color changes, exertional leg pain, or other concerning findings may require vascular evaluation.
Do not assume every foot problem is “just neuropathy.”
Does Exercise Matter?
Appropriate physical activity can be an important part of diabetes management.
Depending on the individual, exercise may support:
- Glucose regulation
- Cardiovascular health
- Strength
- Balance
- Mobility
- Weight management
- Confidence
- Overall health
If neuropathy has significantly affected balance or foot sensation, the exercise program may need to be modified for safety.
The goal is not:
“Exercise as hard as possible.”
The goal is:
Keep moving—but move wisely.
Nutrition Matters, but There Is No Magic Neuropathy Diet
A healthy nutritional pattern can support diabetes management and overall health.
But be cautious of anyone promising that one food, supplement, or restrictive diet will “cure diabetic neuropathy.”
Nutrition should support:

- Adequate protein and nutrients

The best plan is one that is medically appropriate and sustainable.
Be Careful With “Nerve Supplements”

But taking a product simply because the label says “nerve support” is not the same as identifying what your body actually needs.
Remember:
More is not always better.
Excessive vitamin B6 intake, for example, can itself cause neuropathy.

Those accomplishments matter.
The body doesn’t always reward good decisions immediately.
Sometimes the benefits are things that don’t happen:
A complication that doesn’t progress as quickly.
A fall that doesn’t occur.
A wound that doesn’t develop.
A health problem that is caught earlier.
Progress isn’t always dramatic.
But it still counts.
